Immunizations

All incoming students are required to have proof of immunization turned into the school office prior to first day of school.  According to Public Health/Iowa Administrative Code section 641-7.5 (139A) Proof of Immunization, all applicants, or their parents or guardians shall submit a valid Iowa Department of Public Health certificate of immunization to the admitting official of the school or licensed child care center in which the applicant wishes to enroll.  Applicants not presenting proper evidence of immunization, or exemption, are not entitled to enrollment in a licensed child-care center or elementary or secondary school under the provisions of Iowa Code section 139A.9.

Immunization requirements for Kindergarten students:

Diphtheria/Tetanus/Pertussis (DTP or DTaP):  5 doses with at least 1 dose of diphtheria/tetanus/pertussis containing vaccine received on or after 4 years of age if the applicant was born on or after September 15, 2003.

Immunization requirements for 7th grade students:
Meningococcal (meningitis) vaccine:  one dose received on or after 10 years of age for applicants in grades 7 and above, if born after September 15, 2004

Tdap (Tetanus/Diphtheria/Pertussis): a one-time booster dose of Tdap vaccine is required for students enrolling in seventh grade or if born on or after September 15, 2000

Immunization requirements for 12th grade students:
Meningococcal (meningitis) vaccine: 2 doses of meningitis vaccine for applicants in grade 12, if born after September 15, 1999; or 1 dose if received when applicants are 16 years of age or older.

Polio (OPV/IPV):  4 doses, with at least 1 dose received on or after four years of age if the applicant was born after September 15, 2003.

Measles/Mumps/Rubella (MMR):  2 doses of measles/mumps/rubella containing vaccine, the first dose shall have been received on or after 12 months of age; the second dose shall have been received no less than 28 days after the first dose or the applicant demonstrates a positive antibody test for measles and rubella from a U.S. laboratory.

Hepatitis B:  3 doses if the applicant was born on or after July 1, 1994.

Varicella (chicken pox):  2 doses received on or after 12 months of age if the applicant was born on or after September 15, 2003, unless the applicant has a reliable history of natural disease.
